We have an unwritten rule in the observation unit. Having more than 3 cats makes you ineligible for admission to the unit. Whenever we go against our better judgement and accept someone with a cat score of >3, it always ends badly. Problems compound, complaints are filed and unit operations grind to a halt.
Over many years of doing this, we have been fooled by people with less than 3 cats who have not done well in our observation unit. So I have devised an adjusted cat score to capture this demographic. Having more than 3 drug allergies, a diagnosis of fibromyalgia, chronic lymes disease or gall bladder dyskinesis gives you one cat each. You may actually have someone with no cats but has an adjusted cat score of 3.
All this is a bit tongue-in-cheek but sadly speaks to our culture; always in overdrive - destroying peace and turning us into basket cases.
